Insurance Billing
Our offerings may be covered under the Physical Therapy (PT) and/or Occupational Therapy (OT) benefits of your health insurance or employee benefit plan .  We are currently able to bill:

• Auto accident claims
• Medicare (you will need a referral from your physician)
• Regence Blue Shield and other Blue Cross Blue Shield Plans PPO
• First Choice Network
• Triwest
• Out-of -Network providers for many Preferred Provider Organizations (PPO) insurances including Premera, Aetna, Cigna. (Check with your insurance company about your out-of-network co-pays and deductibles.)
• We are NOT able to bill Washington Apple Health/ Medicaid Plans or Labor & Industry

Rates for Insurance Billing

• Billing for is done in 15 minute incriminates at $67.00 per 15 minutes for

          CPT codes 97140, 97530, 97110, 97112, 97116, 97535

• Initial Therapy Evaluation is billed a $120.00 CPT 97161, 97162, 97163 (PT) or 97165 (OT)
  

Rates for Wellness* visits
         *non-insurance, same day payment required, a sliding scale may be offered for low income individuals

• $135.00 55 minute session
• $70.00  initial intake charge - one time only - in addition to the 55 minutes session fee

Payment

• For insurance billing, you will be sent a statement of any amount due after the insurance billing has processed for deductible, co-pay, or co-insurance.  You will be given a link to pay this online or by check.

• Cash or check are preferred for same day wellness services and co-pays.
• We do NOT accept credit cards in the office, however you will see a link to "Kareo" on you billing statement where you may use a credit card for payment.

Cancellation Policy 
We schedule our one-on-one time just for you.
If you do not show up for your scheduled appointment 
and you have not notified us at least 24 hours in advance, 
you will be required to pay for the session in full.
