Terms and Conditions & Liability Waiver
Holistic Physical Therapy and Wellness

By registering online for any class, session, or wellness service provided by Holistic Physical Therapy and Wellness, you acknowledge that you have read, understood, and agree to the following Terms and Conditions and Liability Waiver. Your registration constitutes a legally binding agreement.

 

1. Voluntary Participation and Assumption of Risk
   You understand that participation in exercise and wellness classes involves inherent risks, including but not limited to physical exertion, muscle soreness, falls, or injury. You voluntarily assume full responsibility for any risk of injury, illness, or other loss that may result from your participation.

2. No Medical Advice or Physical Therapy
   Unless explicitly stated, classes and services are intended for general wellness, education, and fitness and do not constitute physical therapy or individualized medical treatment. You agree to consult your physician or healthcare provider before participating, particularly if you have any health conditions, are pregnant, or are recovering from injury or surgery.

3. Release and Hold Harmless
   By participating, you agree to release and hold harmless Holistic Physical Therapy and Wellness, its owners, employees, independent contractors, and affiliates from any and all claims, liabilities, damages, or expenses that may result from your participation. You also agree to indemnify and defend these parties against any claims arising from your actions.

4. Fitness to Participate
   You affirm that you are physically capable of participating in the class or activity. You agree to listen to your body, modify activities as needed, and stop participation if you experience pain, dizziness, or other concerning symptoms.

5. Modifications and Cancellation
   Holistic Physical Therapy and Wellness reserves the right to modify, reschedule, or cancel any class, session, or instructor assignment at its discretion. Efforts will be made to provide notice of changes.

6. Media Release (Optional Clause)
   Unless you notify us in writing, you grant permission for Holistic Physical Therapy and Wellness to use photographs or video taken during classes for marketing or educational purposes.

7. Consent and Agreemen
   By registering online, you confirm that you have read and understood this Terms and Conditions & Liability Waiver in full and agree to be bound by its contents. This agreement remains in effect for all future participation unless revoked in writing.
